REGISTER IN 4 SIMPLE STEPS

  • Provide your email address at check-in here at MCMC.

  • Your invitation will be sent to the email address you provided us at check-in. Check your junk/spam folder if you don’t see it within 24 hours.

  • Click the email link to launch the portal website to register!

  • Create a username and password to complete registration.

VIEW MEDICATIONS, VITALS, IMMUNIZATIONS

  • View/print your immunization lists for school, sports, or travel.

  •  Review vitals from your visit(s).

  •  View/print your medication list.
